Privacy policy

This page explains what Gaze Eye Care collects through this site and in the office, and how we use it. It is a sample for the owner to review with counsel before use.

When you send a message through this site, we receive the name, phone number, email and message you type. We use them to reply to you. Please don't include medical details in a message; call the office instead.

Your health records are kept in the office's records system, not on this site. We keep your records private and share them only as the law allows, for example with your insurance plan to bill a visit or with another doctor you ask us to send them to.

This site has no analytics, advertising pixels, session recording or embedded scheduler. The booking button opens the practice's own scheduling page, which has its own policy.
